ARTWORK PROOF WAIVER

IF YOU CHOOSE NOT TO SEE A PROOF OF YOUR ART, YOU MUST SIGN AND RETURN THIS FORM TO BEGIN PRODUCTION ON YOUR ORDER.

I understand that although my art will go through an extensive pre-flight procedure, by choosing not to receive and review a proof of my art prior to production, I acknowledge that unwanted errors can occur on my final printed product. I understand that the only way for Crystal Clear Disc & Tape to accurately prevent these errors is to send a proof for my approval. If any unwanted errors occur, I understand that this is my responsibility and will not hold Crystal Clear Disc & Tape responsible for any print errors that could have been prevented by my review of a proof. I will accept any incorrect printing "as is" and pay Crystal Clear Disc & Tape the full amount for the final product under the terms of my written price quote.
