This order form is meant simply as a convenient way to communicate the details of your project to Crystal Clear. It is not a final commitment of your order – you can always change or update details as needed with your customer service representative. Of course including accurate and complete information will allow us to process your order most efficiently, however if you are uncertain of any of the choices below, simply choose the closest option or put a “??” in the space provided, or contact us with any questions. Your customer service representative will get with you with any questions, details, or missing information on your order as needed. Note that a company Purchase Order can replace this order for if this is more convenient for you.

IMPORTANT Be sure to include payment for deposit with your order. We cannot begin your order without the required 75% deposit.
